Understanding Multi-Forms

As a system administrator, you can link more than one form template to each incident type and decide which user groups can view and edit each form. This feature allows dispatchers and responders to select the most relevant form from the Forms dropdown list in the Incidents window. Dispatchers or responders to the incident can use an existing form or duplicate a form, for example:

  • Multiple responders in the same incident can each fill out a separate form.
  • Multiple participants in the same incident (e.g., separate forms for each person involved in the same accident) can each fill out a separate form.

Viewing, Editing, and Duplicating Form Templates

As an administrator, you can create incident types. When you create a new incident type, you also assign form templates to the incident type, as well as the groups that can view, duplicate, or edit the form template.

Form Template Name

Each new form is tagged with the name of the form template. Depending on how the system administrator has defined the form, the form name may also contain the text entered in the form title field. In this case, when creating a new form template, the title of the form includes both the form name and after the form title field is filled, the form name inherits its title from the text entered in this field.

For example, the following screenshot shows an incident that has three form templates:

  • Injury Details which contains a field called Name.
  • Injury Details 1 which contains no fields.
  • Suspect which also contains no fields.

If you choose the form that contains the Name field and fill in the required details, the form name changes (after you have saved it) to include the name entered in the field, as shown in the following example.

The name of the creator of each form in an incident is displayed under the form name in the Forms tab. When an incident is opened, all forms that have already been created for that incident type are designated as having been created by the user who opened the incident. If a form template is duplicated, the creator is the user who duplicated the form template.

Required Fields

Form templates can be defined (by the administrator) with required fields. A red asterisk is displayed next to the name of a form that has required fields. The red asterisk is also displayed next to the required fields in the form. Additionally, the red asterisk is displayed next to the form name in the Incidents window.

A dispatcher can only close an incident when all required fields of all form templates have been filled out. A user can only report an incident as Ready if all required fields in the form have been filled out.

How to Use Multi-Forms in the Dispatcher?

To use Multi-Forms in the Dispatcher, follow these steps:

  1. Select an incident type: Click on the dropdown menu on the Dispatcher home page and select the desired incident type.
  2. Fill out the form: After selecting the incident type, the corresponding form appears. Fill in the required information in the form. When the form is correctly filled out, click “Save”.
  3. View Multi-Forms: If the incident type is linked to more than one form, a dropdown menu appears on the form. Click on the menu to view and select all available forms.

How to Use Multi-Forms in the Mobile App?

To use Multi-Forms in the NowForce mobile app, follow these steps:

  1. Select an incident type: Tap on the icon of the selected incident type to open a form.
  2. Fill out the form: Fill in the required information in the form. When the form is correctly filled out, tap “Save”.
  3. View Multi-Forms: In case the incident type is linked to more than one form, a dropdown menu appears on the form. Tap on the menu to view and select all available forms.

How to Add Multi-Forms Permissions?

To add Multi-Forms permissions, follow these steps:

  1. Go to the NowForce administration menu: Log in as Administrator and click on “Menu” in the top right corner of the screen. Select “Administration” from the dropdown menu.
  2. Multi-Forms permissions: Click on the “Permissions” option in the Administration screen. Then click on “Multi-Forms” and check the “Allow” box next to the desired Multi-Forms permission level.
  3. Save: Click “Save” to save the changes.

How to Manage Incident Types and Add Form Templates?

To manage incident types and add form templates, follow these steps:

  1. Go to the NowForce administration menu: Log in as Administrator and click on “Menu” in the top right corner of the screen. Select “Administration” from the dropdown menu.
  2. Incident management: Click on the “Incidents” option in the Administration screen. Select the desired incident type and click on “Templates” to add a Form template for the incident type.
  3. Create form template: Click on “Add Template”, choose the name and description for the template, and click “Save”.
  4. Add form: Add form elements to the template by dragging them to the desired location. Set the required fields and save the template.
